Many insurance plans have certain guidelines and requirements that you as a policy holder and we as specialists have to follow.
These requirements may include:
- Obtaining a referral authorization from your primary care physician.
- Using only designated hospital, lab or X-ray facilities.
- Obtaining your plan's approval for hospitalization or surgery.
- Completing any waiting period for a pre-existing condition.
Your insurance carrier may delay or deny payment that could leave you financially responsible for services not paid or covered by your policy if these guidelines are not followed. If you have any questions regarding your policy's requirements or coverage, please talk with your carrier or our staff.

We also recognize and participate in a number of other plans. Please verify acceptance of your particular plan with your carrier and/or our staff.
Claims will be prepared and submitted to your carrier provided we have all the necessary information. Co-payments, deductibles and any services not paid or covered in full by insurance are the responsibility of the patient. We will be happy to assist with questions concerning your particular coverage, our fees or to set up payment arrangements.
